Job Description:

A company is looking for an Inpatient Oncology Social Worker.
Key Responsibilities

Conduct initial bio-psychosocial assessments and coordinate with the clinic team for cancer patients
Assist with patient-centered discharge planning and identify barriers to discharge
Provide various counseling services and education on resources related to cancer care

Required Qualifications

Master's Degree in social work
Minimum of 2 years of relevant experience
Experience with chronic illness and medical social work preferred
Inpatient experience preferred
5 years or more as LMSW or LCSW preferred
